Project “5G-OR” – Kick-off for the operating room of the future

The project partners of the cross-border Franco-German joint project “5G-OR” have been gathered in Strasbourg to set the course for successful interdisciplinary cooperation. The project aims to create a Franco-German 5G-capable operating room ecosystem for hospitals. After interesting presentations from the fields of medicine, telecommunications, AI, robotics, and medical technology, the participants worked intensively on the realization of the digital hospital.

5G OR Consortium

The partners of the 5G-OR project are developing various 5G applications in the operating environment. The aim is to improve patient care in hospitals. The applications will be deployed in German and French facilities with 5G campus networks and validated in a realistic clinical environment.

Networking medical devices with powerful communication technologies such as 5G enables fast and structured collection of medical data and vital signs parameters.  Aided by AI algorithms, the data can be interpreted close to real-time and can also be used for telemedical applications. In addition, robotic assistance systems can be used to provide support in everyday hospital life and thus relieve the burden on the medical staff. The solutions developed, such as the use of data-controlled and AI-supported surgery, are intended to improve surgical outcomes and increase patient safety. With the help of 5G campus networks, the required technical infrastructure is to be implemented in the operating rooms of the Charité Berlin, the Fraunhofer IPA in Mannheim, and the IHU Strasbourg.

The Institute of Image-Guided Surgery of Strasbourg is pleased to announce the nomination of six directors, as of April 7, 2022, on the occasion of the renewal of the college of qualified persons or representatives of the economic world.

Philippe Richert succeeds Arnold Munnich as Chairman of the Board of Directors. A senator between 1992 and 2010, he was minister in charge of local authorities from 2010 to 2012. He chaired the regional council of Alsace and then the Grand Est from 2010 to 2017 and the Régions de France association from 2016 to 2017.

We are very happy to announce the 1st Surgical Data Science (SDS) Summer School to be held in Strasbourg, France, from 18th – 22nd July 2022.

Recognizing the surge in Surgical Data Science research and the need for effective clinical translation, the SDS summer school aims to promote research and innovation of clinical value by educating clinicians and computer scientists on respective contexts, needs, tools and methodologies.

This first school organized by the Institute of Image-Guided Surgery, IHU-Strasbourg, and the University of Strasbourg will be focusing on endoscopic video analysis. Leading clinical and computers science experts from top institutions will cover subjects related to the clinical use of endoscopy and endoscopic video analysis, spanning from endoscopic instruments and data annotations all the way to advanced deep learning algorithms and the design of clinical translation studies.

Short online lectures on fundamentals of endoscopy and computer science will be freely accessible starting from 15th of March 2022 at: https://edu4sds.eve-evolving-education.eu/. Upon completing these fundamentals, prospective participants can apply to the onsite summer school consisting of a series of lectures, hands-on labs, and group projects. Twenty (20) selected computer scientists and clinicians will have the opportunity join us at Institute of Image-Guided Surgery, IHU-Strasbourg, from 18th – 22nd July 2022 to work in a truly multidisciplinary environment with unique clinical and computer science resources to come up with novel ideas and data science solutions of clinical value.

5G-OR is one of the 4 winning projects jointly selected by the Federal Ministry of Economics and Climate Action in Germany and the Ministry of Economy, Finance and Relaunch in France for the call “Technical developments and application ecosystems for 5G private networks”. The joint Franco-German 5G-OR project takes up the challenge in the healthcare domain. It will explore the benefits of a 5G-enabled operating room ecosystem – a fundamental hospital component – to support medical and paramedical teams to improve patient outcome. The ambition is to strengthen Europe’s position in the 5G-enabled medical technology arena by establishing an innovation ecosystem for the development of new products and quality services.

Towards the hospital of the FUTURE

Digitisation is now being integrated into the modern hospital infrastructure. In this context, wireless communication technology is emerging as a central element of a hospital’s future environment with the need to achieve rapid and secure data transmission and analysis. The goal of 5G-OR is to create a demonstrator of a Franco-German 5G private network application ecosystem for hospitals with a specific focus on the Operating Room (OR) environment. The OR is a highly suitable environment to demonstrate the value of the intra-hospital 5G private network as it is the most concentrated node of healthcare complexity with data-dense and logistics-heavy requirements. In addition, the OR is the highest risk environment for patient safety and a major cost centre for the hospital.

5G-OR is centred around the development of the next generation of a 5G-enabled OR ecosystem intended to improve patient outcome. The underlying concept is that data-driven and Artificial Intelligence assisted surgery within a digital OR will enable more accurate and precise operations as well as fewer complications. The OR generates massive and disparate data from a multitude of sources. Here, the challenge is to make data usable for example by providing real-time feedback to medical staff. The 5G communication technology holds the capacity to enable efficient exploitation of the data generated in the OR. It is expected that efforts towards standardisation requirements for quality service and industrialisation will take the next-generation OR to a new level of service. 5G-OR will assemble these elements into a workable and reproducible package and subsequently validate its worth and interoperability with cross-border clinical use cases.

Four clinical use cases

The consortium has selected four use cases to be operated in the Franco-German 5G ecosystem to demonstrate the value of 5G private networks in the OR. The first two use cases tackle the decision-making in the OR for better patient safety through real-time analysis of massive data. The teams will use Artificial Intelligence to continuously analyse patient vital signs and surgical video, transmitted by 5G wireless medical devices for the detection of early warning signs of potential complications. Another use case pertains to remote-enabled robotic telesurgery powered by real-time data with the goal to perform remote cross-border robotic telesurgery using a 5G network connection. The fourth use case targets the optimisation of the complex logistics of the OR environment. The consortium will evaluate the benefits of 5G-enabled logistics support in the OR by autonomous mobile robotic assistant providing devices, materials, and instruments required for surgery.

The 5G-OR consortium is well balanced between R&D, Healthcare, and business. It is leveraging strong cooperation between complementary French and German partners, who have a common vision of the digital OR for better patient care. This includes hospitals and R&D centres : Institute Fraunhofer IPA in Mannheim, Institute of Image-Guided Surgery in Strasbourg, Institute of Research and Technology b<>com in Rennes, Charité University Hospital in Berlin, Reutlingen University, together with an innovative large company and SMEs : Karl Storz in Tuttlingen, SectorCon in Berlin and RDS in Strasbourg. Partners in 5G-OR are leaders in all aspects needed for the success of the project and have already made significant contributions to solving critical elements of this global vision.

Artificial Intelligence enters the operating room: A world first at the IHU Strasbourg

The scientific and medical teams at the IHU have just taken a major step in the international race towards the surgery of the future. They have successfully deployed an image analysis system driven in real time by computer software using Artificial Intelligence, to automatically control the progress of a minimally invasive operation on a patient.

During the operation performed by Prof. Didier Mutter on 25th November, a minimally invasive gallbladder removal, the video acquired by the endoscopic camera was analyzed in real time by Artificial Intelligence algorithms designed Prof. Nicolas Padoy and his team at IHU/University of Strasbourg. The software can follow all stages of the operation and movement of the instruments, recognize the anatomy, and automatically perform controls of the surgical scene at key steps. All the information can be communicated to the surgical team in the form of notifications or augmented reality images that are completely synchronous with the video of the operating field. The operation was broadcast live from one of the Strasbourg IHU operating rooms on the screens of the Digestive Surgery Congress in Rome and the World Congress of Endoscopic Surgery in Barcelona.

The University Hospital Institute (IHU) of Strasbourg has become the first French facility to obtain recognition of compliance with good laboratory practice for the preclinical evaluation of medical devices.

Recognition by the ANSM

On October 21, following over two years of work, the French National Agency for the Safety of Medicines and Health Products (ANSM) awarded the Image-Guided Surgery Institute of Strasbourg recognition of compliance with Good Laboratory Practices. This recognition concerns implantable medical devices (vascular or digestive stents, surgical clips, prostheses) and diagnostic or treatment assistance devices, both hardware and software, including systems based on Artificial Intelligence.

The IHU in Strasbourg is the first academic institute in France, and one of the very first in Europe to obtain this recognition.

A guarantee for Industry

Certification of GLP compliance sets the IHU of Strasbourg apart from its peers for industry partners and collaborators. It allows them to simplify and accelerate their own accreditation processes, but also to test products before they are approved. Valid on the European market (CE marking), but also in all OECD member countries including the USA (FDA) and Japan, the accreditation represents the Industry’s confidence in trusting the IHU’s preclinical expertise.

An experimental platform and network of experts that are unique in the world

The IHU’s experimental platform is fitted with cutting-edge equipment in the fields of surgery, imaging, and minimally invasive therapies (flexible endoscopy, endovascular or percutaneous treatments, robot-assisted surgery). Since its creation, over fifty industry or academic laboratory professionals have been able to work in this platform in a wide range of fields, from hepato-gastroenterology, to cardiovascular studies, neurology and neurosurgery, as well as urology, interventional pneumology, thoracic surgery, gynaecology, and orthopaedics. By coming to the IHU in Strasbourg, manufacturers benefit from the expertise and tailor-made services of an internationally recognized research institute to test and verify the safety and expected effectiveness of their medical devices.

According to IHU Director, Benoît Gallix, “The recognition of GLP compliance should enable us to double this contracted research for industry professionals within two years”.

We accompany them from the preparation of their research plan by advising them on the experiments most suited to meeting their objectives and go as far as providing them with a complete report that can include a file for submissions to regulatory authorities.

Our experts are personally involved in these studies so as to contribute their knowledge and expertise and may also participate in the improvement of the product or the procedure thanks to their experience feedback.

The services we offer also apply to the organization of training sessions with industrial customers to train practitioners in the use of a new medical device or a new therapeutic approach. More particularly, we have a catalogue of preclinical in vivo and ex vivo models that we can adapt to the needs of our customers (catalogue provided on request) for a tailor-made course, adapted to the specificities of the device.
